LDC Forms
Scuba Diving is an inherently dangerous activity. Read over the form carefully to understand the risks involved in scuba diving.
Scuba diving is an adventure activity that requires the use of specialized life support equipment in an underwater environment where humans could not otherwise exist. As with other adventure activities, scuba diving has elements of risk that cannot be totally eliminated regardless of the amount of training, care, caution or expertise. Ultimately it is up to each individual diver to assume the inherent risk associated with scuba diving and each diver’s responsibility to minimize the risk through exercising good judgment, common sense, respect and personal awareness during all diving activities.
Scuba diving and freediving require good physical and mental health. Fill out your dive medical form with your physician to ensure the health and safety of yourself and others before diving with LDC.
Bring the Diver Medical Guideline to your physician while filling out the Diver Medical Questionnaire. The Guidelines are used by your physician to accurately examine and assess your dive fitness.
All divers joining LDC on a trip to an international or domestic destination must fill out a trip application form. This form includes reservation policy, cancellation policy, and liability release.
